Personal Data Application and Response Procedure
    GENERAL INFORMATION:   In accordance with the Law on the Protection of Personal Data numbered 6698 ("KVKK"), individuals defined as "data subjects" ("Data Owner") have the right to apply to the Data Controller (Jeopark Municipalities Union) regarding their personal data processing and the rights listed in Article 11 of the KVKK. Applications related to these rights must be submitted to our Municipality in writing or electronically, in accordance with the provisions of Article 13/1 of the KVKK and the relevant provisions of the Regulation on the Principles and Procedures for Application to the Data Controller.
  APPLICATION METHODS:
In applications to our Municipality within the scope of the Law on the Protection of Personal Data, according to Article 5/2 of the Regulation on the Principles and Procedures for Application to the Data Controller, published in the Official Gazette dated 10.03.2018 and numbered 30356 and in effect on that date, it is mandatory to have information about the applicant's name, surname, and if the application is in writing, signature; for Turkish Republic citizens, T.C. identity number; for foreigners, nationality, passport number, or if available, identity number; residence or workplace address for notification purposes, if available, the email address for notification, phone and fax numbers, and information about the subject of the request.
  Written Applications:
Written applications to our Municipality must be submitted in person to the Municipal Clerk's Office, Public Relations Directorate, and Human Resources Directorate or sent to the address  "4 Eylül Mah. Yunus Emre Cad. No: 96 Kula / MANISA" via notarized power of attorney showing the authority to apply for the rights specified in Article 11 of the KVKK or by notarized mail.
  • It must be delivered personally or by notary to the Municipal Clerk's Office, Public Relations Directorate, and Human Resources Directorate and sent to the address mentioned above via notarized mail.
Electronic Applications: Electronic applications to our Municipality must be signed with a secure electronic signature defined in the Electronic Signature Law numbered 5070 and, if signed with an electronic or mobile signature, sent to the email address  manisajeopark@hs01.kep.tr via registered electronic mail (KEP),
  • or sent to the email address  info@jeoparkbelediyelerbirligi.com via registered electronic mail (KEP), provided that the relevant person who applied has been previously notified to our Municipality and is registered in the Municipality's system.
  • It must be sent using a software or application developed by our Municipality for the purpose of the application. (At this stage, there is no software or application developed by our Municipality for this purpose yet.)
Your applications submitted to us will be processed promptly and no later than thirty days from the date your request reaches our Municipality, in an effective, lawful, and honest manner. The response will be provided in writing or electronically. In case any deficiencies are identified by our Municipality regarding your application, if these deficiencies are not addressed by you within seven (7) days from the notification, your request will be suspended until the deficiencies are resolved by our Municipality. However, if additional costs are incurred for the process, according to Article 7 of the Regulation on Application Procedures and Principles to the Data Controller, no fee will be charged for up to ten pages, and a processing fee of 1.00 Turkish Lira will be charged for each page beyond ten. It is important that the content of your request is clear, understandable, and determinable in terms of date. Therefore, the information and documents requested by you in this application form must be filled in completely and accurately and submitted to our Municipality through the above methods.

This "Application Form" is prepared to identify your relationship with our Municipality, determine your personal data processed by our Municipality, and respond to your application correctly and within the legal period. In order to prevent legal risks arising from unlawful and unfair data sharing, especially to ensure the security of your personal data and to conduct a healthy examination of your request, our Municipality reserves the right to request additional documents for identity and/or authorization determination or related to your request. Our Municipality does not accept responsibility for requests that are not correct and up-to-date or for unauthorized applications made to our Municipality. In accordance with the requests mentioned above, I request that my application to your Institution be evaluated in accordance with Article 13 of the Law, and I kindly ask for information to be provided to me.
